Domestic Partnership Agreement

A legal document formalizing a committed, non-marital relationship, outlining shared finances, property, health care, and benefits. Ensures clarity and protection for both partners.

FERPA

The FERPA (Family Educational Right and Privacy Act) allows parents or trusted adults to access a student’s education records after age 18, ensuring colleges share grades, attendance, or tuition details. Essential for parents supporting college students.

Financial Power of Attorney (POA)

Authorizes a trusted person to manage your finances, like paying bills, during travel, illness, or unexpected events, ensuring your affairs stay on track when you can’t handle them.

Health Care Power of Attorney (POA)

Appoints an agent to make health care decisions for you if you’re incapacitated. This includes choosing treatments, surgeries, and end-of-life care, ensuring your Living Will medical wishes are honored when you can't communicate them yourself.

HIPAA Authorization Form

HIPAA (Health Insurance Portability and Accountability Act) allows a trusted person to access your private health information or discuss treatment during illness or hospitalization. It allows sharing information, not decision making.

Last Will and Testament

Directs how assets are distributed, appoints an executor, and names guardians for minors after your death, ensuring your wishes are followed. Without it, the court decides. Essential for anyone with family or property.

Living Will or Health Care Directive

Details your medical treatment preferences, if terminally ill or unconscious. Guides health care providers and agents by detailing what medical interventions you do or do not want; it complements a Health Care POA and/or a Medical Authorization Form.

Medical Authorization Form

Authorizes someone to get general health updates from non-HIPAA-covered sources and to make specific medical decisions for another person. Ideal for emergencies but narrower than a Health Care POA.

Revocable Living Trust

Lets you control assets during your life and pass them directly to chosen heirs after death, bypassing costly, public probate. It’s especially helpful if you own a home, have children, or want to simplify estate transfer for family.

Temporary Guardianship Authorization

Allows another adult to care for your child temporarily. Covers school, health, and daily decisions during your absence due to travel, illness, deployment, or any time a parent is unavailable for a period of time.

Temporary Travel Consent Form

Authorizes a minor to travel with a non-parent adult, often needed for domestic or international trips like school outings, family vacations, or visits with grandparents, ensuring smooth travel without parents.
